Did you know you can access up to 5 therapeutic intervention sessions with one of DR Rehabilitation’s Allied Health practitioners via medicare with minimal out of pocket costs to you?
How does it all work?
If you have a chronic medical condition, have a complex care need or are a resident of a residential aged care facility, you can access services via a referral from your GP under the GP Management Plan (GPMP- Item 721) or Team Care Arrangements (TCAs- item 723), as well as Multidisciplinary Care Plan (MCP-item 731) for age care facility residents.
So what is considered a chronic medical condition or a condition that requires complex care?
According to The Department of Health, a chronic medical condition is one that “has been (or is likely to be) present for six months or longer, for example asthma, cancer, cardiovascular disease, diabetes, musculoskeletal conditions and stroke”. One is considered to have complex care needs “if they need ongoing care from a multidisciplinary team consisting of their GP and at least two other health or care providers”.
Group therapeutic intervention services:
DR rehabilitation facilitates multiple group exercise therapeutic intervention services that individuals with type 2 diabetes are eligible to access via medicare. Type 2 diabetics can participant in up to 8 group sessions, including a comprehensive initial assessment facilitated by one of our Exercise Physiologists per calendar year.
How do I go about arranging a referral to DR Rehabilitation from my GP?
If any of the above criteria's relate to your circumstances, we suggest you visit your treating GP, and ask them to assess your eligibility to access services under the above schemes. Where appropriate the GP will be able to provide you with the relevant referral and upon receipt of the referral, we will get in touch as soon as possible to get things started!
